An incorrect phone number or county assignment can prevent someone from finding help. We want to know about errors and changes in the directory, articles, or links.
What you can report
Please tell us if an agency has a new telephone number, changed its address or name, stopped accepting walk-in visits, expanded or reduced its service counties, or changed its official designation. You can also report a broken source link, confusing article wording, or an important outdated program rule.
What helps us investigate
Include the URL of the page, the exact text or listing that seems wrong, the correction you suggest, and—if possible—a public official source showing the new detail. You may provide an agency webpage, a public notice, or the name of the relevant agency to check. We do not need Social Security numbers, medical records, bank statements, or copies of private application documents.
How we handle reports
We review the claim, compare relevant official records, and update the page when the evidence is sufficient. If official sources disagree, we may temporarily label the information uncertain or remove the disputed detail while investigating. More urgent routing mistakes, such as an invalid assistance number, take priority over minor formatting requests.
What a correction does not mean
We cannot change a government benefit decision, grant someone eligibility, move a person ahead on a waitlist, or require a service agency to respond. If you need an official appeal or complaint, contact the decision-making agency using the notice you received.
Send the correction
Use the form below. Set the subject to “Directory correction” and provide a link to the relevant public page.
If you need immediate service assistance
Contact the relevant official local agency directly. The Eldercare Locator and DIAL can help connect you to appropriate networks when a local listing is not available.
